NUHS is committed to enhancing the health of the community we serve, and our Regional Health System works in close collaboration with our community care partners to meet the health and social care needs of the residents and bring community care services closer to home. Together with healthcare providers and community care agencies, we want to empower Singaporeans to take ownership of their own health and wellness. We are in partnership with primary care stakeholders and community partners in the western region of Singapore to co-create a care eco-system of health and social services to support Healthier SG and social prescribing. NUHS provides more holistic and coordinated primary care services, as well as opportunities for partnership and dialogue to foster stronger partnerships between polyclinics, General Practitioner (GPs) and our hospitals. In addition, continuous learning resources and relevant research are made available to GPs and clinic assistants.

The NUHS Community Care Teams comprising community nurses, care coordinators, health coaches and allied health professionals who are sited at Community Health Posts (CHPs) in residential areas to provide residents with easy access to basic health services close to their homes.  These services can be particularly useful for patients requiring transitional care following a stay at one of the acute hospitals.

 

Services provided by our Community  Community Care Teams include the following:

  • Geriatric care and home safety assessments

  • Caregiver training, education and support

  • Case management and coordination

  • Fall prevention, screening and education

 

If your clinic is under a Primary Care Network (PCN), please contact your PCN headquarters for referrals to a Community Health Team/Community Care Team.

Our GP partners in the neighborhood provide residents with the first line of care in the community. They can treat mild to moderate conditions such as cough and fever, help manage chronic illnesses such as diabetes, as well as recommend health screenings and vaccinations when required. 

 

As a NUHS GP Partner under our Primary Care Network, you will have access to valuable support, resources and services to provide quality care to residents. We look forward to working closely with you to build a healthy and engaged population in the West.

The NUHS Primary Care Network (PCN) aims to improve Chronic Disease Management in the western community by bringing together GP partners to deliver team-based care with different community stakeholders. This initiative creates a support network for patients with chronic medical conditions (such as Diabetes and High Blood Pressure) and allow patients to have convenient access to medical services and facilities.

 

The PCN team will develop personalised care plans for patients and facilitate access to medical, social and financial support services as required. Patients will also be encouraged to undergo health checks, learn how to better manage their conditions and mitigate significant lifestyle risks for early intervention.

 

NUHS PCN covers seven regions to better focus on local community needs and provides the following services:

  1. Diabetic Foot Screening (DFS)

  2. Diabetic Retinal Photography Screening (DRP)

  3. Nurse Counselling

  4. Spirometry Services

  5. Care Coordination, basic health coaching, referral to community services

Together with NTFGH, NUH and AH, NUHS started the GPFirst Programme on 1 April 2021. This programme aims to encourage patients to seek attention at GP clinics instead of visiting A&E for non-emergency conditions. Through this, the programme aims to reduce self-referrals to A&E and improve the appropriateness of GPFirst referrals to A&E.

​Benefiting medically stable patients from our Specialist Outpatient Clinics (SOCs), the programme aims to appropriately site them to a primary care partner closer to their homes. For patients with complex conditions, it aims to deliver seamless care through appointment consolidation and management under a primary physician or specialist.

 

Patients with well-controlled chronic conditions and do not require specialist care will be appropriately transitioned from Specialist Outpatient Clinics (SOCs) to a primary care partner in the community closer to home.

 

The key primary care partners under this programme include St Luke's Outpatient Clinic, Family Medicine Clinics (FMCs), National University Polyclinics and private GPs under the Primary Care Network (PCN).
