The National Healthcare Group (NHG) collaborates with General Practitioners and Polyclinics to facilitate patient access to care for acute ailments and the management of chronic conditions. NHG's aim is that each patient is assigned a dedicated Primary Care physician, to ensure seamless patient care integration during transition between community and hospital settings. By ensuring every patient has a designated Primary Care physician, NHG fosters continuity of care and enhance patient outcomes across different healthcare environments. As part of the national Healthier SG plan (Healthier SG), NHG collaborates with GPs to assist residents in achieving their health goals and connects them with community resources such as programmes, activities and services.
