With our care delivery model now focused within the community, General Practitioners (GPs) play an essential role in managing the population's health and social care needs holistically. SingHealth works with primary care centres around fostering private-public partnerships and empowering GPs to provide person-centred and place-based care.
SingHealth values each and every collaboration opportunity and aims to build strong and long-lasting relationships that benefit all our GP partners involved. To strengthen private-public partnerships and facilitate care delivery, SingHealth engages GPs using three levels of engagements at the strategic leadership level, local leadership level, as well as at the ground level.
GP partners are strongly supported by SingHealth through these four key pillars of support (i.e., the 4 Cs) – Community Care Resources, Capability Development, Communication Support and Care Services Support. The goal of SingHealth is to ensure that GPs are well-equipped with the information and resources, capability, communication platforms and clinical services to provide their patients the person-centred and place-based care based on their needs.
