Supported by the Ministry of Health, the Singapore National Asthma Programme (SNAP) was established in 2001, to address the burden of asthma in Singapore.
Since 2022, SNAP has aimed to support asthma care in primary care, starting with the polyclinics, through the identification, tracking, and recalling of patients who should be on regular asthma follow-up, but default on appointments.
