​Community Health Centres (CHCs) are set up in residential areas to provide ancillary health services, to support General Practitioners (GPs) in caring for patients with chronic conditions. CHAS, Merdeka Generation (MG) and Pioneer Generation (PG) cardholders can enjoy subsidised rates for these services.

For better coordination and continuity of care, patients must be referred by GPs. Test results will be sent to the referring GPs for review and follow-up with patients.   All services are by appointment only.


The following is a list of services that may be available at CHCs.

  • Diabetic Foot Screening (DFS)
    Detection of early foot problems in patients with diabetes to prevent complications and reduce the risk of leg amputations. Patients will also receive tips on proper foot care.

  • Diabetic Retinal Photography (DRP)
    Detection of early signs of eye damage from diabetes to prevent complications that can lead to partial or complete vision loss.

  • Nurse Counselling (NC)
    Nurse counsellors work with referring doctors, to advise and support patients through a personalised care plan which, among many things, includes sharing of lifestyle tips and dietary modifications. The nurse counsellor will also support patients in identifying barriers and setting targets to keep their chronic conditions under good control.​

  • Dietetics
    Advising patients on detailed meal planning to suit their medical condition(s), and provision of personalised dietary counselling.

  • Physiotherapy (PT)
    Helps patients with injuries or other conditions to relieve pain, and improve and restore functional ability.

  • Podiatry
    Diagnoses and treats disorders associated with feet and ankles.


Step 1: Patient consults the GP for his or her chronic condition.

Step 2: After assessing and diagnosing the patient’s condition, GPs may refer the patient to a CHC for relevant services by completing the respective CHC's referral form and contacting the CHC to schedule an appointment.

Step 3: On the day of the appointment, a trained healthcare professional at the CHC will perform the referred service(s) for the patient.

Step 4: The healthcare professional will contact the GP should there be any need for escalation or urgent concerning issues.

Step 5: Following the appointment, the report(s) will be sent to the GP for review and follow-up with the patient.
